Consent form – Body Waxing

Please note that waxing does have certain side effects such s skin removal, redness, swelling, tenderness, etc. If you have used or are currently using Accutane, Retin-A, Diferrin or Renova, Tretinoin, Adapalane, Alustra, Avage, Isoretinoin, Avita, Tazarotene we cannot wax you today. You may experience skin sensitivity/ thinning, which can result in skin lifting from the following:

• sunburned skin
• pregnancy
• menstruation
• antibiotics
• certain medical conditions or any medical conditions that are not listed here.

I have read the above information and if I have any concerns, I will address these with my stylist. I give permission to my wax stylist and Glam India salon to perform the wzxing procedure and will hold her nd her staff harmless from any liability that may result from this treatment.I agree that this constitutes full disclosure and that it supersedes any previous verbal or written disclosures. I certidy that I have read and full understood the above details and that I have had sufficient opportunity to discuss any questions I have regarding the procedure. I undertand the procedure and accept the risks. I do not hold the esthetician or Glam India Threading & Spa responsible for any of my conditions that were present but not disclosed at the time of this skin care procedure which may be affected by the treatment performed today.
