Consent Form – Japanese Hair Straightening Many people are allergic to a wide variety of foods and natural substances. Medical experts do not always know what triggers those reactions. Despite the rigorous evaluation and testing done on the keratin treatments we offer, we know that a small amount of people may be sensitive to some of the ingredients used in the formulas. As with any chemical used in the salon, a patch test is always recommended and is of particular use when a client is known to have other sensitivities to chemicals, products or foods. In order to conduct a patch test, a small amount of the product is placed on the inside of your wrist. If you should experience any sensitivity, burning or prickly, tingling sensation, redness or other temporary discoloration of the skin after applying, that is an indication that the product is notsuitable for your particular skin and a keratin service should not be performed for you. I will perform a patch test prior to the treatmentYes , I willNo, I refuse to Full disclosure:All forms of keratin Treatments involve a technical application of chemical formulas at dangerously high heat levels. There are a number of keratin formulas on the market claiming to be “formaldehyde free”. Technically, this may be true. However, even the “formaldehyde free” formulations are likely to contain other “aldehydes”, urea, or similar mixes which share the potent characteristics of formaldehyde. Often, these compounds convert into a form of raw, unrefined formaldehyde when heated and can create potent fumes that are noxious and irritating during the curing process. Your service will be performed in a well-ventilated area for this reason. More importantly, the fumes may be potentially harmful. The “aldehyde” ingredients are necessary for the bonding of the keratin to your hair, and unfortunately there are no exceptions to this. No other bonding agent can provide the remarkable, long-lasting effects of a Brazilian Keratin treatment. Any keratin treatment that is truly without any formaldehyde or any derivatives, will likely only last until the next shampooing. A beautiful flat-iron and blow-out with some nice keratin-based conditioner might last for several days, but this service is not a Brazilian Keratin treatment. I have received a hair analysis and consultation with a curl and Keratin Treatment specialist. I have provided all information regarding previous treatments to my curl and/or color. My stylist has explained to me the process recommended for my hair type, texture, condition, desired results and lifestyle. I have been given aftercare instructions and product recommendation to best care for, preserve and prolong the treatment results. I am not pregnant or nursing, or I have decided to receive a keratin treatment regardless of any potential risk. Asthma/ Any Allergies? Please Specify I understand that Japanese hair straightening treatments are semi-permanent; which can greatly reduce frizz, bulk and curl. Results vary and I have been advised as to the results I personally can expect from this treatment and how to maintain it. Keratin smoothing treatments can produce fumes which may cause irritations in some sensitive individuals. I have been advised that either treatment does contain aldehydes, which may be released as formaldehydes during the curing process, and that these fumes may have a harmful potential. I release my technician or salon from all liability associated with this procedure. There is no guarantee for the bonding time of the eyelash extensions. This salon is not responsible for any technician errors. I understand the aftercare instructions and will do my part to maintain my eyelash extensions.
